Job Description

Salary: Â£42,852 – £44,904 (Inclusive of 7.5% Market Supplement)

Hours per week: 37 hours

Our Hospital Discharge team are looking for an experienced Social Worker who brings empathy, respect, honesty, compassion, and integrity to their practice, and who is committed to delivering person‑centred support built on trust.

You will join an experienced and supportive team where teamwork and wellbeing are central to how we work. Working across three acute hospitals, you will play a key role in providing advice, assessments. and coordinated discharge planning to ensure safe and timely transfers of care. You will complete complex assessments, act as an Investigating Officer in safeguarding work, and apply strong knowledge of the Mental Capacity Act 2005 and the Care Act 2014. Your approach will be rooted in compassion, respect, and person‑centred practice, working closely with individuals, their support networks. and multidisciplinary partners.

You will bring at least two years of post‑qualifying experience, Social Work England registration, and a strong understanding of relevant legislation. You will also support colleagues by offering peer supervision and guidance, contributing to a trusted and positive team culture.

This position comes with a 7.5% market supplement payment. All market supplement payments are subject to review on an annual basis.
